ABSTRACT

<p><b>OBJECTIVE</b>To observe the signal transducers and activator of transcriptions (STATs) protein expression changes and investigate the functional role of STATs pathway in case of high glucose-induced cardiac fibroblasts (CFs) proliferation and collagen deposition in vitro.</p><p><b>METHODS</b>Rat cardiac fibroblasts were isolated from 1- to 3-day-old SD rats, cells from the second to fourth passages were used for the experiment. CFs were cultured in Dulbecco's modified Eagle's medium, supplemented with 5.5 mmol/L glucose (NG), 5.5 mmol/L glucose plus 19.4 mmol/L mannose (OC) or 25 mmol/L glucose (HG) in the presence of absence of STAT1 inhibitor (fludarabine, FLU) and STAT3 inhibitor (S3I-201). After 24 h and 48 h culture in vitro, the proliferation of CFs was measured by 3-(4,5-dimethyl-2 thiazoyl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) assay. After 12 h and 24 h culture in vitro, the production of type I and III collagen was evaluated using real-time quantitative PCR and ELISA. After 0, 30, 60 and 120 min culture in vitro, the phosphorylated expression of STAT1 and STAT3 was analyzed by Western blot.</p><p><b>RESULTS</b>CFs proliferation was significantly enhanced post 24 h and 48 h HG stimulation, and procollagen I and III mRNA expression was significantly upregulated post 12 h and 24 h HG stimulation. Deposition of collagen I and III was also significantly increased post 24 h and 72 h HG stimulation. STAT1 phosphorylation in CFs was increased after 120 min HG stimulation and STAT3 phosphorylation in CFs was increased post 60 min and 120 min HG stimulation. FLU and S3I-201 could inhibit HG-induced CFs proliferation and suppress of which was stimulated by FLU and S3I-201 could both suppress upregulated procollagen I and III mRNA expression and the deposition of collagen types I and III post HG stimulation. STAT1 phosphorylation inhibition resulted in less mRNA downregulation of procollagen type III than that of procollagen type I post 12 h HG stimulation. The STAT3 phosphorylation inhibition resulted in more significantly upregulated procollagen type III mRNA expression than procollagen type I mRNA expression at 12 h post HG stimulation.</p><p><b>CONCLUSION</b>HG could enhance the protein expression of phosphorylated STAT1 and STAT3 in CFs, which are responsible for HG-induced increased CFs proliferation and collagen deposition in vitro.</p>

ABSTRACT

[Summary] Vitamin D is one of the fat-soluble vitamins which,eventually through activation in the liver and kidneys within the human body,becomes an important hormone——1,25-dihydroxyvitamin D3.Not only may 1,25dihydroxyvitamin D3 promote calcium and phosphate absorption by the intestine,but also stimulate the macrophages to secrete a protein(cathelicidin) which is able to digest the microbacterias,including viruses and bacteria,playing an antiinfective effects.Theofore,a proper supplementation of vitamin D would be beneficial for human health.

ABSTRACT

A total of 144 patients with Graves disease were divided into 3 groups according to their disease courses (≤ 1 year,1 year < course ≤ 5 years,> 5 years).The bone metabolic biochemical markers and bone mineral density (BMD) of 144 patients with Graves disease and 26 normal controls were observed.Compared with control group,serum levels of calcium,phosphate,alkaline phosphatase and urinary levels of phosphate and magnesium in Graves disease patients were significantly increased(P < 0.01 or P < 0.05),but serum levels of magnesium and 25 (OH) D3,1,25 (OH)2D3 were significantly decreased (P < 0.01 or P < 0.05).BMD of all sites in patients were significantly lower than that of normal controls (P < 0.01 or P < 0.05).

ABSTRACT

Objective To assess the effects of artificial cordyceps sinensis(Jin shuibao) on the numbers of podocytes and epithelial-mesenchymal transition in diabetic rats.Methods Diabetes was induced by intraperitoneal injection of low dose streptozocin.Sprague-Dawley rats were randomly divided into three groups:control group (CON),diabetic model group(DM),and artificial cordyceps sinensis treatment group(CS).After intervention for 8 weeks,the numbers of podocytes were estimated hy immunohistochemistry.The expressions of nephrin,desmin,and monocyte chemoattractant protein-1 (MCP-1) were detected.Results Compared with control group,DM group showed significantly decreased podocyte numbers and nephrin mRNA,with increased desmin and MCP-1 levels(P<0.05).While the numbers of podocytes and nephrin mRNA were increased,and the expressions of desmin and MCP-1 were decreased in CS group as compared with those in DM group (all P<0.05).Conclusions Artificial cordyceps sinensis notably improves epithelial-mesenchymal transition of podocytes,and increases the number of podocytes in diabetic rats.

ABSTRACT

Pituitary hyperplasia may be found in patients with primary hypothyroidism as the decreased thyroid hormone level attenuates negative feedback effect.Sometimes the enlarged pituitary may be misdiagnosed as a pituitary tumor.Patients with long term untreated hypothyroidism often have extremely high level of serum creatine kinase and thus may be misdiagnosed as suffering from myositis.In order to increase the awareness of the nonspecific symptoms of primary hypothyroidism,this article introduces the diagnosis and treatment of a patient with primary hypothyroidism with raised serum creatine kinase level and pituitary hyperplasia.

ABSTRACT

ObjectiveTo investigate feasibility of use of glucose injection instead of glucose powder in oral glucose tolerance test (OGTT). MethodsSixty healthy adult volunteers without history of diabetes were recruited for a standard OGTT with 75 g anhydrous glucose powder first. One week later, they were randomly divided into two groups, each of the one group (30 volunteers) orally took seven ampoules (20 ml/ampoule) and each of the other group (30 volunteer) took 7.5 ampoules of 50% glucose injection for OGTT again, as compared to those with standard OGTT.Plasma levels of glucose and insulin were examined to evaluate whether different forms and dosage of glucose had similar results in OGTT. ResultsIn 23 volunteers with normal glucose tolerance, their plasma levels of glucose were ( 4. 8 ± 0. 4 ), ( 6. 7 ±0. 6), (5.9 ±0. 8), (5.5 ±0. 9) and (4. 8 ±0. 9) mmol/L at 0, 30, 60, 120 and 180 min after oral load with 75 g anhydrous glucose powder, respectively. These values changed to (4. 8 ± 0. 3 ), (7.5 ± 1.1 ),(6.8±1.8), (6.3 ±1.0) and (4.6 ±1.2) mmol/Lor (4.7 ±0.3), (7.2±1.3), (6.1 ±1.1),(5.6 ± 0. 9 ) and (4. 3 ± 0. 9) mmol/L after oral load with seven ampoules ( 15 volunteers) or 7. 5 ampoules of (8 volunteers ) of 50% glucose injection, respectively.With standard OGTT, 37 cases of impaired glucose tolerance were found from 60 volunteers tested, and their plasma levels of glucose were (5. 2 ±0.6), (9. 1 ±1.4), (8.9 ±2.6), (6.7±2.0) and (4.7 ±1.0) mmol/L at 0, 30, 60, 120 and 180 min after oral load with 75 g anhydrous glucose powder, respectively; (5. 1 ± 0. 7 ), ( 8. 8 ± 1. 7 ), (9. 0 ±3.0), (7.3±2.2) and (5.1 ±1.1) mmol/L (15 volunteers) or (5.3 ±0.6), (8.8 ±1.9), (8.5 ±2. 4), (6. 6 ± 1.4) and (4. 8 ± 1.6) mmol/L (22 volunteers) at 0, 30, 60, 120 and 180 min after oral load with seven or 7.5 ampoules of 50% glucose injection, respectively, with no statistically significant difference between varied methods.Normal serum level of insulin was found in 38 of 60 volunteers, with their logarithmic transformation of serum insulin levels of 1.5 ± 0. 3, 3.9 ± 0. 3, 3.7 ± 0. 4, 3.2 ± 0. 6 and 2.2 ±0. 8 at 0, 30, 60, 120 and 180 min, respectively after glucose load in standard OGTT, and 20 of 38 volunteers with normal serum insulin of 1.7 ± 0. 4, 3.9 ± 0.4, 3.4 ± 0. 7, 3.3 ± 0. 8 and 2. 4 ± 0. 7 at 0,30, 60, 120 and 180 min after oral load with seven ampoules of 50% glucose injection, respectively, or 18 of 38 with normal serum insulin of 1.7 ± 0. 4, 3.9 ± 0. 4, 3.8 ± 0. 5, 3. 3 ± 0. 7 and 2. 3 ± 1.0 at 0, 30,60, 120 and 180 min after oral load with 7. 5 ampoules of 50% glucose injection, respectively, with no statistically significant difference between varied methods. Twenty-two cases of high serum level of insulin were found from 60 volunteers with standard OGTT, with their logarithmic transformation of serum insulin of 2.2±0.6, 4.7 ±0.5, 4.9±0.7, 4.2 t 1.0 and 2. 8 ±0.9 at0, 30, 60, 120 and 180 min after oral load with 75 g anhydrous glucose powder, respectively; 10 of 22 volunteers were found with high serum insulin level of its logarithmic transformation of 2. 4 ± 0. 6, 4. 7 ± 0. 5, 4. 7 ± 0. 3, 4. 1 ± 0. 8 and 2. 8 ± 1.1 at 0,30, 60, 120 and 180 min after oral load with seven ampoules of 50% glucose injection, respectively ; and 12 of 22 volunteers were found with high serum insulin level of its logarithmic transformation of 1.9 ± 0. 5,4. 5 ± 0. 6, 4. 6 ± 0. 6, 3. 7 ± 1.0 and 2. 4 ± 0. 9 at 0, 30, 60, 120 and 180 min after oral load with 7. 5ampoules of 50% glucose injection, respectively; with no significant difference between varied methods.There also was no statistically significant difference in occurrence of adverse effects between these three OGTT methods. ConclusionsEither seven or 7. 5 ampoules of 50% glucose injection can substitute 75 g anhydrous glucose powder in OGTT, with similar test results and safety.

ABSTRACT

Parathyroid adenoma is the main cause of primary hyperparathyroidism and often associated with thyroid nodular goiter.Thyrothymic thyroid rest belong to the ectopic thyroids which are classified into 4 grades according to the state of their connection with the proper neck thyroid gland.Thyrothymic thyroid remnant can also develop into nodular goiter and may be difficult to be distinguished from parathyroid adenoma.We present herewith the diagnosis and treatment of a rare case of parathyroid adenoma accompanied by thyrothymic thyroid remnant nodular goiter in order to remind clinicians of the attention to the thyrothymic thyroid remnant disease.

ABSTRACT

To investigate the regulation of osteoclast inhibitory lectin (OCIL) mRNA expression by prostaglandin E2 ( PGE2 ) in rat osteoblastic cells and the involved signaling pathways. Rat primary osteoblasts and UMR106 osteoblast-like cells were cultured and treated with various doses of PGE2 or regulators of different signaling pathways for different periods of time, the cells were then harvested at indicated dates. Total RNA were isolated and OCIL mRNA expression were studied by real-time PCR. PGE2, Forskolin, db-cAMP, and A23187 increased OCIL mRNA by 2. 38 fold,4. 2 fold,4. 5 fold, and 5. 1 fold ( all P<0. 01 ) respectively, while PMA downregulated OCIL mRNA expression by 50% ( P<0. 01 ). KT-5720, verapamil, W7, and PD98059 downregulated PGE2 induced OCIL mRNA expression by 56%, 40%, 65%, and 60%, respectively( all P<0. 0l ). While chelerythrine enhanced PGE2 induced OCIL mRNA expression by 30% ( P<0. 05 ). PGE2 up-regulated the expression of OCIL in rat osteoblastic cells via PKA, MAPK, and Ca2+/Calmodulin signaling pathways.

In patient with primary biliary cirrhosis,the metabolism of calcium and vitamin D could be affected and osteomalacia and secondary hyperparathyroidism might occurr.Besides,hypoalbuminemia may mask the real level of serum calcium and thus lead to misdiagnosis of coexisting parathyroid adenoma.Therefore,a rare case of parathyroid adenoma associated with primary biliary cirrhosis was herewith presented to call attention to the effect of hypoalbuminemia on serum calcium.

ABSTRACT

Objective To study the bone turnover and its related molecular mechanism in STZ-induced diabetic rats. Methods Of 30 male SD rats studied, 15 were induced diabetics by intravenous injection of streptozotocin (50 mg/kg)and fed for 8 weeks. After the sacrifice of both the diabetic and control groups, serum Ca, P, alkaline phosphatase (ALP), and osteocalcin were determined, and 24 h urinary Ca and urinary cross-linked N-telopeptide of type Ⅰ collagen (NTx)and creatinine (Cr)ratio were also determined. The left tibia was dissected for bone histomorphometry analysis. Right femur and lumbar vertebrae (L1-L4) were reserved for bone mineral density (BMD) determination. The right tibia was separated for the study of bone tissue RANKL/osteoprotegerin, Core binding factor 1 (Cbfa1) ,osterix and osteocalcin mRNA level which was performed by real-time quantitative reverse transcription polymerase chain reaction assay. Results No significant difference was found in serum Ca, P, and ALP levels between 2 groups of rats. ST-Z-induced diabetic rats were characterized by extreme hyperglycemia, marked weight loss, polyuria, and hypercalciuria. A low-turnover osteopenia was evidenced in diabetic rats by decreased BMD in both femur [(0. 099±0.013) vs (0. 139 ± 0.013 g/cm~3) , P < 0.01] and lumbar vertebrae [(0. 107±0.011)vs (0. 149±0.009) g/cm~3, P<0.01] , reduced serum osteoealcin level [a marker of formation, (3.03±0.52) vs (6. 18±0.71) ng/ml ,P<0. 01]) ,decreased urine NTx/Cr ratio [(5. 67±0.86) vs (5.23±0.98) nmol/g Cr, P<0. 05], decreased trabeeular volume and thickness, and reduced bone label surface and bone formation rate [(0. 44±0. 11) vs (0. 78±0. 14) μm/d,P<0. 01] by bone dynamic study. The RANKL/ osteoprotegerin [(0.57±0.11)vs (0.89±0.13) ,P<0.01] ,osterix [(1.93×10~(-4)±0.65×10(~-4))vs (4.19×10~(-4)± 0.71×10~(-4)) ,P<0.01] ,Cbfa1 [(26.68×10~(-4)±6.53×10~(-4))vs (37.21×10~(-4)±7.14×10~(-4)) ,P<0.01] ,and osteocalcin [(2.25×10~(-4)±1.19×10~(-4))vs (3.43×10~(-4)±1.63×10~(-4)) ,P<0.01] mRNA expressions were declined in the bone tissue of the tibia in the ST-Z-induced diabetic rats, as compared with the control. Conclusion A low-turnover osteopenia is evidenced in STZ-induced diabetic rats by significant decrease of both osteoclastic marker(RANKL/ osteoprotegerin)and osteoblastic marker (osterix ,Cbfa1 ,osteocalcin)mRNA levels in tibia.

ABSTRACT

The effects of cyclosporine A on matrix metalloproteinases(MMP)-2 and-9 expressions in the cardiac muscle of streptozocin-induced diabetic rats were investigated.The expressions of MMP-2 and MMP-9 mRNA and proteins in the cardiac muscle of diabetic rats were significantly upregulated,which could be decreasedby cyclosporine A(P<0.05 or P<0.01).The results suggest that cyclosporine A may effectively ameliorate the injuries of diabetic cardiomyopathy.

ABSTRACT

Objective To analyze the clinical characteristics of five Sj(o)gren's syndrome (SS) patients with severe ostalgia. Methods Five SS patients from the endocrinology department of our hospital were retrospectively analyzed. Results All the patients were female, with 9.5 year average course of disease, obvious ostalgia, muscle weakness and complaint, limitation of activity, and different degree of exocrine gland impairment. All the patients had different degree of acidosis, disorders of calcium and phosphorus metabolism, and dyspoiesis of calcitriol. Among them, three patients were found to have hyperinsulinemia and muscle pathological changes detected by the biopsy. Conclusions System factors must be investigated when patients present the bone symptom. At the same time, muscle problem should not be neglected and the patients should be treated as an entirety.

ABSTRACT

Objective To study the autoimmune injuries on diabetic retinopathy(DR)and the protective effects of immunosuppre8sive therapy with eyelosporine A(CsA) on the DR of streptozotocin(STZ)-induced diabetic rats.Methods STZ-induced diabetic rats were randomly divided into 3 groups: group 1:diabetic group without any treatment;group 2:insulin-treated group;group 3:CsA-treated group which was further divided into 2 subgroups:subgroup A:CsA was given 1 week before hyperglycemia appeared and subgroup B:CsA was given one week after hyperglycemia appeared.Subgroup A and subgroup B were further subdivided into 3 groups respectively,based on the dose of CsA(1,4 and 8 mg·kg-1·d-1).As a control group,normal rats were also simultaneously monitored.The pathologic changes in the retina were investigated with HE stain and the deposition of immunoglobulins was detected with immunohistochemistry and immunofluorescent microscopy.Results After 8 week,the deposition of IgG, IgA and IgM was quite significant in the retina of diabetic rats.The data also suggested that insulin treatment had no effects on the DR.In contrast,with CsA intervention,the deposition of immunoglobulins on the retina of diabetic rats vanished.Conclusions Autoimmune injuries were shown,in the present study,to play a critical role in the pathogenesis of diabetic retinopathy.Immunosuppressive treatment with CsA showed protective effects by inhibiting the deposition of immunoglobulins in the retina of diabetic rats.

ABSTRACT

BACKGROUND: As a multifunctional cell growth factor, vascular endothelial cell growth factor (VEGF) plays various biological roles under some physiologic and pathologic conditions. OBJECTIVE: To summarize molecular structure and biological effects of VEGF. RETRIEVAL STRATEGY: A computer-based online search of PubMed database was undertaken to identify related English articles dated from January 1989 to December 2007 with keywords"VEGF, molecular structure, biological effect, diabetes, diabetic macroangiopathy". 934 articles were firstly collected, including 38 about molecular structure and biological effects of VEGF, and 896 about diabetes and diabetic macroangiopathy. After the first trial, only articles about ①molecular structure and biological characteristics of VEGF, and ②diabetes and diabetes complicated by coronary heart disease or limb vascular disease were selected. 897 repetitive, outdated and unrelated articles were excluded. Finally, 35 articles were included. LITERATURE EVALUATION: Of 35 articles, there were 17 ones about animal trials, and in vivo, in vitro and cytological studies, 8 of review articles and comments, and 10 clinical studies. All articles were individual evaluation. DATA SYNTHESIS: Human VEGF genes consist of 8 exons and 7 introns, and are located in 6p21.3 region. VEGF encodes several isoforms with different biological characteristics. Two VEGF receptors display different biological effects. VEGF and its receptors are regulated by various factors. The receptors interact with ligand and play biological effect. In diabetic patients complicated by coronary heart disease, myocardial VEGF mRNA and protein expressions are increased but the expressions of two kinds of receptors are decreased compared with non-diabetic patients. Compared to patients with non-severe lower limb ischemia, VEGF production is more in diabetic patients complicated by severe lower limb ischemia. It is demonstrated that VEGF can restore injured neovascularization. However, VEGF therapy for diabetes still needs further study. CONCLUSION: VEGF can restore impaired neovascularization. VEGF expression shows diversity under diabetic macroangiopathy. Further study should be made focusing on the prevention value of VEGF to diabetic macroangiopathy.

ABSTRACT

Objective To investigate the effects of OGP (10-14) and its derivate H86A on bone metrology in rats with ovariecto-my-reduced osteoporosis. Methods Thirty-two female Sprague-Dawley rats, three months old, were randomly divided into 4 groups. Rats in groups 1 to 3 were bilaterally ovariectomized(OVX) and the others in group 4 were sham-operated. After the operation, rats in group 1 and group 2 were received derivate of OGP(10-14) H86A daily at different concentration. Group 3 were given placebo. Rats were killed after 12 weeks. Left tibias were disconnected and made into non-decalcified bone slice, and then they were detected and analyzed on indexes which were relative with bone metrology. ObjectiveTo investigate the kidney protective e ffects of losartan in diabetic rat and its kidney protective mechanism. MethodsThe rats were randomly divided into the following groups:norm al control (group C), streptozotocin diabetic (group DM) and diabetic rats t reated with losartan (group DL). After eight weeks study, the mRNA expressions of transforming growth factor ? 1(TGF ? 1), TGF ?type Ⅱ receptor (T? ObjectiveTo investigate whether the good control of fasting plasma glucose(FPG) has any effects on bone turnover rate in type2 diabetes mellitus (DM).MethodsA series of markers,such as bone specific alkaline phosphatase (BAP),deoxypyridinoline (DPD),were assessed before and after glycemic control for more than 18 days in 43 poorly controlled type 2 DM patients with initial hemoglobin A1c over 8%.ResultsBy either oral hypoglycemic agent or insulin administration,or both, ideal control of FPG resulted in a reduction in the secretion of urinary calcium(Ca),phosphate(Pi) and magnesium(Mg),and an increase in serum Ca and Pi without obvious changes in serum Mg.The marker for bone resorption,urinary DPD and the marker for bone formation,BAP in serum were reduced significantly before and after the management.The results indicated that the decreases both in DPD and in BAP were not only correlated well with each other,but with the improvement in glycemic indices as well.ConclusionGood control of hyperglycemia in poorly controlled type 2 DM may have the high bone turnover rate back to normal and protect type 2 DM patients from bone loss.

ABSTRACT

Objective To examine the association between the insertion/deletion polymorphism in intron 4 of ecNOS gene (ecNOS4b/a) and diabetic nephropathy (DN) in the Han people of Tianjin, China. Methods The genotypes of ecNOS4b/a polymorphism were detected by PCR-minisatellite DNA detection technique. 70 healthy volunteers, 48 patients with type 2 diabetes without DN, 35 patients with type 2 diabetes and DN without chronic renal failure (CRF), 45 patients with DN in type 2 diabetes with CRF, and 58 patients with non-DN CRF were investigated. Results (1) One case (a female DN patient without CRF) with a rare genotype, the heterozygote of 447 bp and 420 bp, was found. (2) The frequency of allele a in type 2 diabetes without DN group was higher than that in control nonsignificantly (x2 = 1. 672, P = 0. 196) . (3) The frequency of allele a in DN group was lower than that in type 2 diabetes without DN group nonsignificantly (x2 = 1. 082, P = 0. 298). (4) The allele frequencies were similar between the DN without CRF group and the DN with CRF group (Continuity correction x2 = 0. 002, P = 0. 967). (5) The frequency of allele a in DN with CRF group was lower than that in non-DN CRF group significantly (x2=4. 360, P = 0. 037). Conclusions (1) Allele a is not a risk factor of DN in the Han people of Tianjin; (2) The frequency of allele a is lower in the Han people of Tianjin than that in Japanese(4. 3% vs 10. 1% , x2 = 4. 898, P = 0. 027); (3)There are different roles of ecNOS in DN-induced CRF and non-DN CRF.

Objective To investigate the relationship between HLA DRB1 gene polymorphism and susceptibility to leukopenia in patients with Graves′ disease (GD).
